Our Factual Investigation Process

We deliver thorough and legally sound investigations that provide senior management with clear, evidence-based findings. 


Our investigation process: 

  • Scope definition: Confirm terms of reference, objectives and criteria, stakeholders, and information-handling or privilege arrangements.
  • Evidence collection: Secure and catalogue evidence; review documents, records and media; conduct interviews; and inspect relevant locations.
  • Analysis: Reconstruct the timeline, assess causal and contributing factors, and evaluate the control design and effectiveness against WHS obligations (including HVNL CoR, where relevant). 
  • Findings: Test and corroborate evidence, apply procedural fairness, and document verified findings.
  • Reporting: Deliver a clear report with verified findings, contributing factors, and opportunities for improvement. Practical recommendations are included where requested.
  • Senior management briefing (optional): Focused debrief to support decisions, corrective actions, and due diligence.
