Workplace Investigations

Independent, factual investigations by a licensed investigator

When a workplace incident or significant near miss occurs, senior leaders require a clear and objective account of what happened, why it happened, and what must change to mitigate the risk of recurrence.

Organisational Risk Consulting conducts independent, evidence-based investigations that are thorough, fair, and legally defensible. Our verified findings withstand regulatory scrutiny and client review, support risk management and due diligence,  and provide senior leaders with the clarity required to make informed decisions.

Organisational Risk Consulting is a licensed provider of factual investigation services.

What We Do

We conduct thorough workplace investigations into incidents and near misses across diverse operations, grounded in accuracy, procedural fairness, and legal defensibility.

Typical matters include:

  • Notifiable incidents and regulator interest
  • Serious injuries, dangerous occurrences, or significant near misses
  • Patterns or systemic issues that indicate control gaps

Our methods combine document and evidence review, site inspections, timeline reconstruction, and interviews. The outcome is an independent and defensible narrative of events with verified findings. Where requested, proportionate recommendations are provided for consideration by senior management or legal counsel.

What Senior Leaders Gain

  • Independent Perspective: Licensed investigators deliver fact-based findings free from internal bias or conflicts of interest.
  • Clarity and evidence: Verified accounts of events, causes, and contributing factors, supported by a secure chain of custody.
  • Defensible findings: Findings informed by legal training and extensive WHS leadership experience, suitable for executive consideration or legal counsel review.
  • Compliance visibility: Clear line of sight to WHS obligations, supporting informed due diligence and regulator engagement where required.
  • Proportionate improvements: Targeted opportunities to reduce recurrence risk, with optional recommendations and senior management briefings where requested.
  • Executive confidence: Independent reports that support timely, informed leadership decisions.

Our Factual Investigation Process

We deliver thorough and legally sound investigations, providing senior management with clear, evidence-based findings.

Learn more about our investigation process below:

  • Scope definition: Confirm terms of reference, objectives and criteria, stakeholders, and information-handling or privilege arrangements.
  • Evidence collection: Secure and catalogue evidence; review documents, records, and media; conduct interviews; and inspect sites.
  • Analysis: Reconstruct the timeline, assess causal and contributing factors, and evaluate the control design and effectiveness against WHS obligations (including HVNL CoR, where relevant).
  • Findings: Test and corroborate evidence, ensure procedural fairness, and document verified findings.
  • Reporting: Deliver a clear report with verified findings, contributing factors, and opportunities for improvement. Practical recommendations are included where requested.
  • Senior management briefing (optional): Focused debrief to support decisions, corrective actions, and due diligence.

Investigation Principles

We apply disciplined investigation principles to ensure fairness, legal defensibility, and reliable findings based on the following:

  • Independence and objectivity
  • Evidence-based conclusions drawn from verifiable sources
  • Procedural fairness for all participants
  • Confidentiality and secure information handling, with a documented chain of custody
  • Professional interviewing, guided by trauma-informed principles to minimise distress and avoid re-traumatisation.
