Investigation and Compliance Review Following a Heavy Vehicle Incident

Client Context

A Queensland Government Department (name withheld) required an independent workplace investigation into a serious heavy vehicle incident. This heavy vehicle incident investigation needed to address workplace health and safety obligations, Heavy Vehicle National Law (HVNL) Chain of Responsibility (CoR), and transport compliance requirements.


Project Overview

  • Documentation Review: Examined internal investigation reports, training and induction records, safe work procedures, and HVNL CoR documentation.
  • Stakeholder Interviews: Undertook factual interviews with relevant parties to capture multiple perspectives.
  • Factual Reporting: Prepared and presented a comprehensive investigation report with evidence-based findings and actionable recommendations.
